Submitting Course Reports and Requesting Completion Cards
When Do I Submit a Course Report and Issue Course Completion Cards?
Trainers must submit payment for course completion cards and the Course Report Package within seven (7) days of the completion of the class. See the detailed instructions below. Upon receipt of the payment and the Course Report Package, Global OSHA is responsible for issuing course completion cards to students.
How Do I Submit a Course Report and Issue Course Completion Cards?
1. Submit Payment for the Course Completion Cards
In order to submit a Course Report Package, the trainer must first submit payment for the course completion cards through the Payment Processing page. Please enter the quantity of cards requested based on the number of students in the class. Then enter the desired payment method. After payment is completed, please note the invoice number. The invoice number must be included in the course report and email subject line.
2. Prepare the Student Photo Zip Folder
To submit a request for course completion cards, trainers must provide a headshot photo for each student. These photos may be provided to the trainer in advance by the student, or taken by the trainer during the class using a camera or smartphone. The photo files must be labeled with the name of the student, placed in a single folder, and zipped for submission. The instructions for zipping the student photo folder are as follows.
Locate the file or folder that you want to zip.
Press and hold (or right-click) the file or folder, select (or point to) Send to, and then select Compressed (zipped) folder.
A new zipped folder with the same name is created in the same location. To rename it, press and hold (or right-click) the folder, select Rename, and then type the new name.
The Student Photo Zip Folder must be labeled as follows: Trainer Last Name, Course Number, Course End Date, Invoice Number.

3. Complete the Course Report Form
Trainers must complete all course and student information required in the Course Report form, including the payment invoice number. The exact file name for each student photo must be included in the Photo File Name Column. If any name in the Photo File Name Column does not exactly match the actual name of the photo file, the card for that student will not be processed. The trainer will bear the cost of the replacement card. It is the trainer's responsibility to verify and certify the accuracy of all information submitted in the Course Report form.

4. Submit the Course Report Package
Trainers then send the completed Course Report Package containing two attachments (Course Report form and Student Photo Zip Folder) via email to: firstname.lastname@example.org
The body of the email must include the trainer name and trainer ID number, the course number, location (city, state, province, country) and date. The email subject line must be labeled as follows: Trainer Last Name, Course Number, Invoice Number

4. Course Completion Card Distribution
Course completion cards will be sent directly to the students by Global OSHA, using the address provided by the trainer. If the card is returned to Global OSHA as undeliverable, the trainer will bear the cost of the replacement card. It is the trainer's responsibility to verify and certify the accuracy of all information submitted in the Course Report form.
